Eyelid Surgery

by Dr. Morello. If you are in your 30's or 40's, you may have already noticed your upper eyelids starting to sag. Puffy "bags" may be forming beneath you lower lids as well (these are often inherited). As you age further, the sagging may increase and wrinkles develop. The excess skin is trimmed from the upper eyelids and puffiness removed from the lowers. The eyelid shape is not changed, unless you desire a slight angulation at the corner, which can be done. When necessary, tightening of loose or rounded lower eyelids restores their original shape. For some patients with puffy lower eyelids, surgery can be done without a scar (transconjunctival approach), and is sometimes done in conjunction with laser resurfacing to improve the skin texture. Blepharoplasty is usually done under a twilight anesthesia in our office facility, and has a 6-8 day recovery time to looking passable. After surgery your eyes appear bigger and brighter, projecting an alert, rested and more youthful appearance. This is the third most popular cosmetic surgery procedure in the United States.
BROW/FOREHEAD LIFT The most important concept in doing forehead/brow lifting is subtlety. Almost every patient expresses concern about not wanting their brows "too high" after surgery. That is because they have seen numerous people with a surprised look. Clearly that is undesirable; fortunately it is totally avoidable in Dr. Morello's hands. The whole thing is about judgment, aesthetic sense, balance and harmony. Today 95% of our brow lifts are by the endoscopic (minimal incision) method. With traditional brow lifts, patients disliked or feared the long scar, the loss of sensation in the scalp and the surprised look that often resulted. Endoscopic brow lifting overcomes each of these problems. Only several tiny incisions are used, sensation remains intact for the most part, and the surprised (overcorrected) look is a thing of the past. This is another example of modern technology favorably influencing plastic surgery. But some patients are still concerned about having a full brow lift. For them Dr. Morello offers a "Minimal Incision Lateral Brow Lift" which maintains the eyebrows at the original level, but produces the effect of lifting the lateral part of the brow and dramatically refreshing the eye area. This is often done at the same time as eyelid plasty, and we also do it when someone has previously had their eyes done but needs a refinement or refreshing of the appearance of the eyes.
